If your name, home address, phone number or email address changes, please let us know.

It is important that we have up-to-date addresses and contact information for you.

Please inform us of a change of address immediately. Failure to do so, could result in your name being removed from our list because you can’t be traced via correspondence. The practice will be unable to prevent this action taking place.

We may ask for documents to prove certain changes, such as changes of names and sometimes address changes.


Please complete this form (Change of Details Form) and return it to the practice, by posting through the practice letter box, post or email.

You can also ask for one of these forms at reception if you are unable to print a copy.


Patients with hospital referrals must also contact the hospital, to inform them of the change of address.
