New Patient Registration (Students Only)
The practice registers large numbers of students every year and we work closely with Cardiff University to ensure that students are able to see a doctor locally whilst they are studying. We register students living within our practice boundary (Cathays, Roath, Heath, Gabalfa, Lakeside and Cyncoed) and the halls below:

Please use this map to make sure that you live in our catchment area before registering with us - if you live outside of the boundary, we are unable to register you.

New Patient Registration (Non-Students)
The practice welcomes new patients living within the practice boundary. The area covers the whole of Cathays and a large percentage of Heath and Roath. The boundary does stretch to parts of Gabalfa, Lakeside and Cyncoed. The map can be viewed in the main waiting area in the practice and a receptionist will assist in establishing whether or not you reside within our boundary. You can also check this map to make sure that you live in our catchment area.

We strongly recommend that you register as soon as you move into the practice area and not wait until you have a medical problem. We urge students to heed this advice because one day you may need a doctor urgently and it makes sense to have your medical records inclusive of full past medical history in the place where you reside and not left with your family doctor where your parents live.
When you register with us, you must provide 2 forms of ID: one photo ID such as a Passport, Driving Licence or Student ID card and the second should be proof of your address e.g. rental agreement, utility bill, phone bill or residency card. Without proof of ID or address, you will not be able to register at this surgery.
In addition you will also have to complete a registration form in the absence of a current NHS Medical Card and a Health Form which will provide us with a basic knowledge of your past medical history whilst waiting for the arrival of your medical records from your previous practice.
The receptionist will provide you with information regarding the services we offer and will help you with the registration procedure if required. You may make an appointment to see a doctor or a nurse on registration if you have a medical problem or require a medication review.
Your NHS medical card with NHS number will be posted to you from a central office in Pontypool some 6-8 weeks after registration. If you do not receive it, please contact ‘Shared Services Partnership’: Tel No. 01495 332000.
